MCAS is considered a clinical diagnosis. While lab tests can be helpful, they are not definitive and are often normal. Mast cell mediators fluctuate, and many reactions are missed by standard testing. Diagnosis usually relies on symptom patterns across systems, exclusion of other causes, and how symptoms respond to treatment.
